You may not make more than two requests within a twelve month period. Upon receiving such information and disclosures request, we will confirm receipt of the request within 10 days.
Additionally, you may designate an authorized agent to make such a request under the CCPA on your behalf. A designated authorized agent may also use the phone number or the links above to make such a request on your behalf.
To verify such information requests you will need to provide certain information. For requests made by an authorized agent, the authorized agent will need to provide proof that they have your signed permission to submit the request, and we may require you to verify your identity directly and/or directly confirm that you provided the authorized agent such permission. We will confirm the information you provide with the information we have on file to verify such requests. We cannot fulfill your request to provide personal information if we cannot verify your identity or authority to make the request on another’s behalf and confirm that the personal information relates to you. Making a verifiable request does not require you to create an account with us.
To the extent required by law, California residents have the right to request deletion or correction of their personal information collected by Axcelus. However, the right to deletion does not apply to personal information that is exempt or subject to an exception under the CCPA. To submit such a request, you may use the toll-free number listed above or fill out this form and send your request to CA-DataRequests@axcelus.com. The process for verifying such deletion or correction requests is similar to the process to verify information requests listed above.
You have the right not to be discriminated against for exercising any of the privacy-related rights to which you are entitled under applicable law. Axcelus will not discriminate against California residents who exercise their privacy rights conferred by the CCPA.
